Lack of Health Insurance & Legal Access

  • No fixed address, ID, or bank account prevents registration for statutory (GKV) or private (PKV) insurance.

  • Undocumented individuals and some EU migrants fall outside coverage systems.

  • Without insurance, even basic treatment becomes inaccessible.

Overburdened & Fragmented Health Services

  • Public clinics and hospitals are overstretched; emergency rooms serve as last resort.

  • Long wait times for mental health, detox, and specialist care.

  • Services are poorly integrated: healthcare, shelters, and social support operate in silos.

Mental Health, Addiction, and Trauma Ignored

  • High rates of trauma, mental illness, and substance use among unhoused people.

  • Most services require stability, paperwork, or sobriety — often unrealistic prerequisites.

  • Lack of trauma-informed, long-term, and culturally sensitive care.

Social Insurance & Reintegration Gaps

  • Health access is tied to formal employment and residence.

  • Without access to pension, disability, or unemployment insurance, long-term recovery is blocked.

  • Reintegration pathways (rehab, housing, employment) remain inaccessible.

Stigma, Discrimination, and Legal Barriers

  • Homeless individuals face mistreatment, exclusion, and fear when accessing care.

  • Legal status and lack of interpreters further marginalize migrants.

  • System navigation is nearly impossible without external advocacy.
